Please review the Ohio 4-H Achievement Requirements and Instructions before you begin filling out the achievement record. The application is available in two formats, Microsoft Word and pdf. If you download the Word format, you can save your completed application for use in the future. The Older Youth Packet Information is also provided as a reference, if needed. Completed Ohio 4-H Achievement Records are due to the Extension office by 4:30 p.m. on December 3, 2007. Please call the Extension office, at 695-1455, with any questions.
